Application and Final Report Forms

World Community Service Application Form, Interim and Final Reports.

World Community Service (WCS) Applications are for "BRICKS AND MORTAR" type International projects (such as schools, clinics and small water projects) where the amount needed is less than the $5,000 minimum required by TRF.  These projects are funded through an annual per member assessment to D5580 Rotarians through District dues.  An Interim and/or  Final Report must be filed with the Grants Subcommittee for any WCS Project.

Click on the appropriate link to download the WCS Application Form, or Final Report Form.  Interim Reports should be a typed summary of the project status, along with an indication of its completion date and a date when the Final Report can be expected.


District Simplified Grant Application Form and Final Report Forms.

District Simplified Grant Funds can be used for community and International projects with a District dollar for dollar match.  D5580 applies for $25,000/year in DSG Funds, and the applications for funding cannot exceed the currently approved amount of $10,000.  The Grants Subcommittee will review applications on a first-come, first served basis; however, the amount approved by the DGS may be adjusted, depending upon available funds remaining.

TRF (DDF) Grant Application, Interim and Final Report Forms.

TRF Applications are for international projects other than "Bricks and Mortar" type projects such as water projects, educational supplies or health related items where the amount needed is more than $5,000. These projects are matched by TRF at 100% on the District match and 50% on the Rotary Club amount given. Therefore, the minimum total amount for a TRF Grant Application is $12,000 and the minimum amount funded by a Rotary Club (or if partnering with other Rotary Clubs) is $3,400.  An alternative, recently implemented through Future Vision, allows clubs to send their contributions for a project directly to the Rotary Foundation Annual Programs Fund.  In turn, the District will assume the contribution and match it dollar for dollar.  That total amount will then be matched dollar for dollar by TRF.  This enables clubs to more fully leverage their funds and also gives credit to the Club and District toward Annual Program Fund goals.

All Applications must include a budget.  Applications for International Projects must include a letter of request from the host Rotary Club, along with a map specific to the area of the project (or GPS coordinates) and floor plans.
